All accidents and incidents that happen to employees and non-employees on school premises, including contractors and members of the public, must be reported using the online forms below.
The line manager should fill in the form within 24 hours of the accident.
Members of staff are responsible for informing of all incidents to their line manager, and it is then the manager’s responsibility to establish all circumstances and complete the online form.
Note: this form does not have a ‘save as you go’ function; therefore, the following information is required prior to completing:
1. All details of the injured person
2. All details of when and where the incident took place
3. All details of what took place, including causation
4. The extent of any injury sustained
5. First aid, if known
6. Medical treatment, if known
7. Witness details, if known
8. Perpetrator details, for violent incidents
9. RIDDOR report reference, if applicable
10. Other external authority reporting, if applicable.

What happens next?
Once you've submitted an online report, a copy of the form is automatically sent to:
• Team manager
• Health and Safety Unit.
Reporting incidents to the HSE
If the incident also needs to be reported to the Health and Safety Executive (HSE), as required under RIDDOR, please complete the relevant form on the HSE's site.
When reporting under RIDDOR, please also provide the Health and Safety Unit with a copy of the completed HSE report form and reference number.
